Insurance and Policies

Thank you for visiting. We do have a little housekeeping to discuss:

Advanced Integrative Care operates as a private pay medical practice and, as such, we do not participate in any health or medical insurance plans, therefore we do not provide CPT codes for insurance billing.

Patients are responsible for covering the cost of their medical services directly and payment is due in full at the time of service.

  • Advanced Integrative Care is considered an Out-of-Network Provider and a Non-Assigned, Opt-Out Medicare Advantage Plan.
  • Payment is required in full upon each visit. Cash, credit/debit, and HSA cards are accepted.
  • We accept ACH transfers using your checking account information, without a processing fee. 
  • Medication refill requests between office visits must be requested by the pharmacy. Requests may be faxed to 716-803-8568.
  • 72 hours are required for prescription refills.
  • Providers may still require an office visit for refill requests.
  • Controlled medications require an in-person office visit for prescriptions to be authorized. 
  • If the appointment is canceled, any prescriptions that need a refill will be denied until an office visit is completed.
  • Appointments are structured for the time required.
  • Consultations and Follow-Up Care: 30-60 minutes.
  • Therapies and Treatments: 1-3 hours.
  • Please arrive 15 minutes prior to your scheduled appointment time to complete any additional forms that may be required.
  • If arrival to Advanced Integrative Care is fifteen minutes past the scheduled appointment time, rescheduling may be necessary.
  • Patients who “No Show” a scheduled office visit, will be charged a $75 fee using the debit or credit card on file. All future appointments will need to be prepaid.
  • Same-day appointments are offered for significant changes in condition.
  • Any patient wishing to cancel or reschedule an appointment will need to do so 2 business days prior to the scheduled appointment.  As a reminder, our office is open Monday through Thursday from 8am to 4:30 PM.  Please keep in mind that once the 2 business day window has passed, no-shows and last-minute cancellations will be charged to a $75 cancellation fee for provider visits.  
  • If urgent issues arise after hours, call 911 or proceed to the nearest Urgent Care or Emergency Room.

Patients

  • Any patient wishing to cancel or reschedule an appointment will need to do so 2 business days prior to the scheduled appointment. If an appointment is not canceled within that timeframe, a $75 fee will be charged using the debit or credit card on file.

First Appointment Consults​

  • Our investment in new patients begins before they walk through the door. Consequently, consult deposit fees for appointments canceled and not rescheduled are non-refundable.

As a reminder, our office is open Monday through Thursday from 8am to 4:30 PM.

All treatments and test results are provided during an office visit. These are not relayed by phone or fax.

In a perfect world, we would see our patients in our offices for an exam/consultation. We understand that there are times when medical attention is required, but an office visit is not possible. In such conditions, we are pleased to offer telemedicine. You would discuss your specific needs and requirements with your provider. Situations in which telemedicine technology might apply include:

  • Laboratory and Radiology Test Results
  • Medical Cannabis Follow-ups
  • Out-of-Town Patient Conversations
  • New therapies will not be initiated, existing plan of care altered, or prescription changes made via telephone.

Advanced Integrative Care is a HIPAA-compliant practice. A copy will be provided upon request.

Although we treat “the whole person” there are just a few areas in which we do not practice. When possible, we can help patients find appropriate providers for the following:

  • The attending physician when in the hospital. We will discuss care with the hospitalist and provide necessary records to facilitate the best possible outcome.  We will also see patients within 24 – 48  hours after discharge to keep you on your road to recovery, answer any questions you and your family have, and coordinate any post-recommended hospitalization services.
  • Department of Transportation Physicals.
  • Pregnancy Care. We will refer you to an OB/GYN provider.

For the convenience of our patients we do provide in office phlebotomy for a nominal fee which includes drawing and preparation.  Your insurance company will be billed directly from the laboratory company for processing.

We offer several means of communication in order to stay connected with our patients including phone/voicemail, text message, email, and the power to practice patient portal. All communications will be addressed within 72 hours. We thank you for your patience. If there is an emergency during office hours please call the office directly and speak with one of our representatives who will be able to assist you. As a reminder our office is open Monday through Thursday from 8am to 4:30 PM.
